What to Bring      

When you come to the clinic for the first time, one of our team members will ask you for the following:

1. Your insurance card.

2. Your prescription for physical therapy from your caregiver.

3. Any Exam reports such as X-rays or MRI’s.

4. Any applicable co-pay.
